Required Registration Documents
We are looking forward to a great season! Our on line registration for the 2010 season will be open on Monday, April 19th, 2010 at noon for returning athletes. Open registration for the 2010 season will begin on Friday, April 23, 2010 at 7PM. Athletes will be added to rosters on a first-come, first-served basis and as space is available. In the meantime, there are several required documents that need to be submitted. Please take this opportunity to download and complete these forms so they are ready when you register on line.
If you are volunteering as a coach or board member for the 2010 season, please download, print and legibly complete this background check authorization form. It must be returned to the Athletic Director, Robert Brown, with a photocopy of a valid driver's license.

To complete registration for your youth athlete for the 2010 season please mail the ALL the following documents to "Southern Marin Youth Football, PO Box 763, Mill Valley, CA 94942 by June 25th: Note: Incomplete forms or forms not submitted by the deadline may result in forfeiture of the athlete's place on the roster. Please ensure that all forms are completed and that supplementary documentation is also returned by the deadline of June 25th.
- Birth Certificate - 3 copies of the athlete's birth certificate should be provided on an 8.5 x 11 inch page. IMPORTANT: If your athlete did not play football last year our new league (AYF) is requiring a second piece of verification of your child's age. This can be in the form of a copy of a passport, a medical card that includes your child's birthdate or a school document (transcript) that includes your childs birthdate. You may also submit an original birth certificate with a visible seal which will be returned to you after player certification on August 8th. Specifics on the birth certificate:
- This document should be a copy of an official government birth certificate provided by the state or country.
- International birth certificates require an official translation if they do not provide English descriptions.
- An official court document for name change must accompany a birth certificate listing a name no longer in use.
- Hospital or baptismal announcements are not acceptable.

- This document must be completed by a licensed medical professional.
- Make sure the licensed medical professional signs (or stamps) and writes the date on the form. page 2.

- Please fill out every field in this form.
- One parent must sign and date the bottom of this form.
- 2010 AYF Registration Contract Form - (click here to download.) Specifics:
- It is NOT necessary to complete page 1 of this form. This page will be completed by the association.
- Please fill out every field on page 2 of this form (with the exception of the gray area, reserved for official use only).
- One parent/guardian must sign the bottom of this form and initial in 5 places. The player must initial the Helmet Waiver.

- Please review this form with your athlete. It stipulates the players' code of conduct and also the important role that a parent plays in support of the athlete and the team.
- Both the parent and the athlete are required to sign the form.

- All athletes must submit 3 copiesĀ of the 2009-10 End of Year Report Card.
- Only an end-of-year report card is acceptable. Please don't mail us a Fall or Winter report card.
- If the report card is larger than 8.5 x 11 inches (e.g., it is legal-sized), it should be reduced to an 8.5 x 11 inch sheet of paper to fit in the certification notebook.
- A full-year report card (e.g., all 3 trimesters) must be provided for consideration for academic honors.
- The report card must have the full name of the athlete as well as the name of the school clearly visible.
- The report card must have the school year (e.g., "2007 to 2008") clearly visible.
- If the GPA is printed on the report card it must be used for scholastic monitoring.
- A GPA below 2.0 requires scholastic monitoring.
- Medical Insurance Card - A legible copy of the athlete's medical insurance card is required. Specifics:
- The front and back of the card must be copied onto the same side of a single sheet of paper.
Please help out our volunteers who process your paperwork:
- Make a copy of all of the documents you mail to us in case we misplace them.
- Please apply adequate postage. Every year several sets of documents are returned by the USPS with "Postage Due".
- Please do not staple any of the documents. You may use paperclips to keep them together.
- Do not use certified mail or any service which requires a signature; otherwise your documents will be returned.
- Do not use a stiff cardboard mailer. The post office will not put it in our post office box and your documents will be returned.
